Single view of patient

A single view of your patient enables reduced administration and costs, increased clinical efficiency and improved healthcare delivery.

Single view of patient

These are challenging times for the National Health Service. For the foreseeable future it must rise to the challenge of providing first rate patient outcomes and clinical effectiveness while operating with constrained resources. It must find efficiency savings while responding to Government plans to achieve a more patient centric health service.

“Information, combined with the right support, is the key to better care, better outcomes, and reduced costs.”

Equity and Excellence: Liberating the NHS.

Ministers have emphasised the Government’s priorities of making the patient the starting point of everything, including health service funding through the NHS Commissioning Board and GP Commissioners.

Thankfully, the NHS is awash with patient information and data upon which to build these patient centric initiatives. The challenge now is how to unlock the power and potential of these key assets across Primary & Secondary care. As a result of multiple patient care disciplines, departments and locations, patient data currently resides in many individual silos on multiple sites making this a very difficult and time consuming challenge to overcome without a trusted patient index.

What does Entity SVP do?

Entity’s SVP is designed specifically to identify the same patient across disparate systems and connect those records together. This is a key requirement in healthcare organisations where different systems configured in different ways with different types of patient data abound across distinctly different healthcare disciplines and sub-disciplines.

Entity SVP creates a central index where you can find data about any patient and furthermore identify the source systems. Entity SVP keeps existing systems in place with no need to change, remove, or replace anything, enhancing existing investments in technology.

Entity SVP uses sophisticated matching algorithms to link records for the same patient. It incorporates various fuzzy matching and searching techniques to determine whether Mrs. Clare Smith at Erwin Road and Mrs. Claire Smith Irwin Road are one and the same person. This is important when that same patient may have appeared in numerous care situations over a number of years, have changed addresses, or, indeed, have different versions of her name or surname.

Transform into a truly patient-centric healthcare organisation

Imagine the power of becoming a truly patient-centric healthcare organisation. Our solution enables your staff to improve the delivery of care by identifying the patient in real-time, regardless of location, accurately link and locate demographic information, ensuring that the right care is delivered to the right patient at the right time and provide transparency to costs and patient outcomes.

By creating and accessing a trusted patient index using Entity SVP, patient-centric departments are able to reduce time spent navigating between systems and applications, making it easier for staff to use IT resources as well as making clinicians more efficient in their interactions with patients. This in turn improves the quality, safety and efficiency of patient care.

Furthermore, a trusted single view of patient data could act as the catalyst and the foundation upon which to build other key initiatives such as clinical and patient portals or care pathway redesign across multiple healthcare organisations.

How SVP does it

SVP provides cross-organisation patient-centricity by enabling the following:

Pre-configuration for optimal Patient Indexing

  • Patient data model based on experience
  • Entity SVP comes with algorithms optimised for standardisation, comparison and weighting of patient data.
  • Bundled events for common actions, including Admission Discharge Transfer (ADT) across inpatient and outpatient environments
  • Defined data load processes

Proven standards-based integration

  • Integration toolkit for loading patient data
  • HL7 v2 and HL7 v3 support
  • Web services in support of SOA environments
  • Support for Integrating the Healthcare Enterprise (IHE) HL7 v2 & v3 Profiles: –
    • Patient Identifier eXchange (PIX)
    • Patient Demographic Query (PDQ)
    • Audit Trail and Node Authentication (ATNA)
    • Patient Admission Management (PAM)

Efficient data stewardship, governance and data quality resolution tools

  • Collaborative data stewardship
  • Be alerted to data quality issues with custom business rules
  • Locate and correct duplicates
  • Identify and resolve patient data quality issues via preconfigured analytics

Scalability and accuracy

  • Search on current and historical attributes for accurate and confident results
  • Scale to meet the needs of your organisation, regardless of size or scope from under one million to over a billion records
  • Know who accessed data with robust auditing capabilities

How a single patient view enhances patient-centricity

Ultimately, patient-centricity stands or falls on the quality of the patient data you hold across your systems. Without knowing who your patients are, you cannot provide the care they need, when they need it.

Entity’s SVP accurately matches and links patient identities to create a single view for use by electronic medical records, clinical portals, administrative applications and analytics.

It helps to achieve the following goals:

  • Recognise the patient at all points of service
  • Improve quality of care by accurately locating and linking demographic information
  • Reduce costs with data quality resolution tools for reconciliation of patient and provider identities
  • Improve clinician satisfaction by providing more trusted and accurate patient identification at the point of service
  • Govern information sharing policies and patient privacy preferences
  • Accelerate time to value by implementing a proven Patient Index with demonstrated integration capabilities
  • Create a foundation for growth with a scalable master data management solution

Experience

A customised variant of this solution was implemented across a network of 60 hospitals and other facilities nationwide. Entity’s SVP solution enabled it to create an Enterprise Master Patient Index (EMPI), as well as linking together PACs and RIS systems so that the entire healthcare ecosystem (hospitals, radiology, etc) could manage a consistent and accurate view of the patient on a national basis. This has significantly reduced data quality issues, enhanced patient safety and resulted in greater patient satisfaction when booking and managing their relationship with the hospital group and its radiology departments.
